Discharge Planner:

Exodus Recovery Inc, is currently looking for a Discharge Planner to practice in a new Mental Health Urgent Care Center to be located proximate to LAC USC Hospital in East Los Angeles.


Requirements:
Master’s Degree preferred and two (2) years clinical experience in the mental health field. Strong background in Chemical Dependency treatment required. Strong verbal and written communication skills. Strong knowledge of assessment, community resources and crisis intervention skills. Ability to work independently and shows mature judgment.


Job Duties:
Serves as a primary discharge planner for Urgent Care Center clients.
Assists client in the development of social service and discharge plans.
Provides complete documentation of all plans and other case management activities.
Works with the clinical team, client and outside referral sources to plan the referrals that are most appropriate to the client’s needs.
Participates in daily client report meetings and serves as clinical resource for unlicensed staff.
Acts as community liaison for linkage to client referrals.
Maintains current and complete clinical records.


Additionally we are looking for another discharge planner with the following qualifications:

Requirements:

      Nurse Practitioners (NP)

      Requirements:

      Must have a valid CA NP license. CPR required. Two years clinical practice in psychiatry as a Nurse Practitioner with strong emphasis working with chronic mentally ill and co-occurring disorders.

      Job Duties:

      Evaluates and determines clients’ need for medications; dispenses, and administers medications as necessary.  Furnishes medications under the supervision of a psychiatrist and follows standardized procedures. Monitors medication compliance and response to medication.

      Performs psychiatric evaluations; screens for health-related concerns and work with program staff refers to appropriate physical health care provider agency when necessary.

      Completes initial health screening for all enrolled members of the assigned program(s).

      Participate as an active member of treatment team that reviews, discusses and determines the course of treatment for clients.  In this capacity, provides consultation and guidance for treatment planning.

      Assesses the psychiatric needs (and medical conditions that may impact psychiatric conditions) for enrolled members of the assigned program(s).

      Evaluates members for suicidal or homicidal tendencies and inappropriate responses indicating psychosis, anxiety and/or depression, and determines whether the client’s condition is severe enough for referral to a higher level of care.

      Registered Nurses (RN)

      Requirements:

      Must have a valid CA RN license required. CPR required. Two years clinical practice in psychiatric care with strong emphasis working with chronically mentally ill and dually diagnosed clients.

      Job Duties:

      Provides nursing care directly or indirectly to the mental health client whose condition may be unstable and ever changing. Emphasis is placed upon the effective assessment of client needs, effective available resources, delivery of mental health services to include: crisis intervention, medication support, targeted case management, individual rehab, family and group services. In addition, the RN only can also provide individual psychotherapy.

      Functions as a client advocate with all other care providers.

      Works collaboratively with the Exodus physician and nurse practitioner to provide a comprehensive medical system of recovery based services.

      Provides ongoing and spontaneous education to all clients to promote a healthier lifestyle and make better choices for self care.

      Licensed Psychiatric Technicians (LPT)

      Requirements:

      Must have a valid CA LPT license required. CPR required. Two years clinical practice in psychiatric care with strong emphasis working with chronically mentally ill and dually diagnosed clients

      Job Duties:

      Provides nursing care directly or indirectly to the mental health client whose condition may be unstable and ever changing. Emphasis is placed upon the effective assessment of client needs, effective available resources, delivery of mental health services to include: crisis intervention, medication support, targeted case management, individual rehab, family and group services. Functions as a client advocate with all other care providers.

      Works collaboratively with the Exodus physician and nurse practitioner to provide a comprehensive medical system of recovery based services.

      Mental Health Workers

      Requirements:

      Must have a valid driver’s license. CPR preferred. Two (2) years clinical experience in the mental health field.  Strong verbal and written communication skills.  Ability to work independently and show mature judgment

      Job Duties:

      Coordinate and provide transportation for clients to services, appointments, meetings, discharge placements, shelters and activities via program vehicles. Assists in providing a safe, clean, comfortable and therapeutic environment Assists clients with varied activities - example paperwork, planning activities, leisure activities. Assist clients to maintain a safe environment. Assists with meals and dining room assignments.

      Orders and may administer IM psychotropic medications to clients. Monitors clients’ vital signs and symptoms to ensure that clients are not experiencing medical complications requiring

      Case Manager I

      Requirements:

      Must have a valid driver’s license. CPR required. Three (3) years clinical experience in the mental health field.

      Job Duties:

      Serves as primary service coordinator for Department contracted clients available on a 24-hour basis.

      Assists client in the development of personal service plans.

      Facilitates client groups as indicated.

      Assists clients in applying for Social Security and SSI benefits.

      Acts as community liaison. Assists clients with housing resources and other placement.

      Maintains current and complete clinical records.

      Possess knowledge of all Department programs forms and requirements to insure proper client enrollment and continuation in the program.

      Case Manager II

      Requirements:

      Must have a valid driver’s license. CPR required. Must be licensed as an RN, PhD, PsyD, LCSW, MFT, or CAADAC.

      Five (5) years clinical experience in the mental health  

      Job Duties:

     Serves as primary service coordinator for program clients available on a 24-hour basis.

          Assists client in the development of personal service plans.

                   Facilitates client groups as indicated.

      Assists clients in applying for Social Security and SSI benefits.

      Rotates 24-hour primary crisis intervention service within the scope of practice (licensed personnel).  As appropriate, obtains LPS designation.

      Rotates facilitation of treatment team meetings and serves as clinical resource for unlicensed staff.
